$175 – Initial Intake

$150 – per 50 minute session

Insurance

I am currently Out of Network.

Connected Mind Counseling will provide clients with a receipt for services to send to their insurance companies, for reimbursement of services. Please note that reimbursement of services using out of network benefits will vary based on each individual’s insurance policy. We recommend you contact your insurance company to review your out of network coverage plan.

The following questions will assist your inquiry related to your health benefits:

• Do I have Out of Network behavioral health benefits?
• What is my coverage amount per therapy session?
• What is my deductible and has it been met?
• Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
• Is approval required from my primary care physician?
• Is authorization required from my insurance company prior to my first appointment?

Contacting your provider will help verify how your plan compensates you for psychotherapy services.

Payment

I accept all major credit cards as forms of payment.  Fees will be charged the day of service.

Cancellation Policy

If you are unable to attend a session, please make sure you cancel and provide at least 24 hours notice.
A cancellation fee of 85. dollars will be charged for same day cancellations.

Good Faith Estimate – No Surprises Act

In accordance with the law that was put into place January 1, 2022, you have the right to receive a “Good Faith Estimate,” explaining how much your mental health care will cost.

Under the law, health care providers need to give clients who are not using insurance an estimate of the expected charges for psychotherapy services.

You have the right to receive a Good Faith Estimate for the total expected cost of psychotherapy services.   A Good Faith Estimate can be provided before you schedule a service.
